For Christians, the crucifixion and what followed it — worn as a statement of faith rather than decoration. Outside that, it's often inherited: a confirmation gift, a grandmother's, something that came with a memory attached. Both are legitimate reasons to wear one, and nobody can tell which is which by looking.
